The Process:
Step 1: Complete an incident report
Step 2: Send the incident report to HR@commonhouse.com
Step 3: Seek medical care if necessary.
Step 4: Provide your insurance card to the urgent care facility/hospital. TELL THEM IT’S FOR WORKERS’ COMP.
Step 5: Provide them HR’s contact information. HR@commonhouse.com.
Step 6: Do not make a payment of any kind.
Step 7: Send all paperwork received at the medical provider to HR@commonhouse.com

Employees have 30 days from the date the incident occurred to seek medical care. But in order to file a claim an incident report MUST be filed within 48 hours or the claim cannot be verified and will not be covered.

YES! Whether you are hurt or not, an incident/near miss report must be completed.

Because in the event you start to feel negative symptoms/impacts from what wasn’t thought to be an incident, you won’t be eligible for that care.

If a bill is received, please forward this to Human Resources. Do NOT pay the medical bill.

Send a copy of the completed incident report to your manager and to Human Resources.
